$5,100 in Free Money for Americans
Thousands of Americans across the country will receive an amount of up to $5,100 in free money from a $3.75 million data health breach settlement from Magellan Health. Claimants are former and current employees and consumers whose information was affected.
In a published article in The U.S. Sun, they can claim two claims: the Common-Fund Settlement whose Social Security numbers were affected and the Claims-Made Settlement Class whose Social Security numbers were not affected.
How to File a Claim?
According to a report published in Top Class Action, claimants should have received a postcard instructing them on what type of claim they will file. They also need to provide proof of purchase such as:
- telephone or cellphone bills
- gas receipts
- postage receipts or bank account statements reflecting out-of-pocket expenses
Furthermore, those who want to claim need to submit the form before December 21 or mail the completed form so that it is postmarked no later than December 21.
